Tips for Requesting Prescription Renewals Before you request a prescription renewal, please note: Prescriptions will be sent electronically or called in to your specified pharmacy within 48 business hours of my office receiving your request. Please include complete information in your request. Otherwise, we may need to contact you via secure messaging to obtain further information. Be sure to provide all the information requested on the Prescription Renewal Request Form, including your pharmacy information, dose and how often the medication is taken. In the "Dose/How Often" box on the Prescription Renewal Request form, please include: Times taken per day Amount taken per dose Form of the medication, such as tablet, capsule, or liquid. For example: 2 capsules 2x/day This feature is for established patients only. You must have been seen in our office within the last year to request prescription renewals online. Emergency prescription requests must be called directly to our office or to your pharmacy.
